Direct answer: Tirzepatide is a once-weekly prescription injection sold under different brand names and indications. In eligible participants, trials reported substantial average weight loss alongside lifestyle support. Results vary, contraindications apply, and safe care requires a long-term strategy—not merely access to a pen.
This guide is general health information. It does not advertise or sell a prescription medicine, show prices, or guarantee a prescription, product supply or personal result.
1. What are Mounjaro and tirzepatide?
Once-weekly medicine
Tirzepatide is a long-acting subcutaneous injection. Mounjaro is a brand, not a generic term for every weight-loss injection. The product registered in Thailand, its approved indication and its current leaflet determine how it may be used.
GIP and GLP‑1 action
It activates both receptors, affecting glucose-dependent insulin release, glucagon, fullness and the speed at which the stomach empties. Reduced appetite can lower energy intake, while delayed gastric emptying can contribute to nausea and interactions with some oral medicines.
How it differs from Ozempic, Wegovy and Saxenda
Semaglutide in Ozempic/Wegovy is a weekly GLP‑1 medicine; liraglutide in Saxenda is generally a daily GLP‑1 medicine. Tirzepatide also targets GIP. Brand names and indications are not interchangeable, and more receptor targets do not automatically mean better suitability.
Mounjaro versus Zepbound
Both contain tirzepatide, but approved uses and product labels vary by country. A clinician must verify the locally registered product rather than relying on overseas social-media descriptions.
2. How much weight loss did research find?
SURMOUNT‑1 at 72 weeks
In adults with obesity or overweight plus a complication and without diabetes, mean weight changes were −15.0%, −19.5% and −20.9% for 5, 10 and 15 mg, compared with −3.1% for placebo. All groups received lifestyle intervention. These group averages cannot predict an individual's first month or final result.
SURMOUNT‑5 versus semaglutide
At 72 weeks, mean weight reduction was approximately 20.2% with maximally tolerated tirzepatide and 13.7% with maximally tolerated semaglutide. The trial does not make one drug universally preferable; contraindications, tolerability and access remain decisive.
Changes beyond body weight
Trials reported improvements in waist circumference, glucose, blood pressure and selected lipids. Anyone taking medicines for diabetes or hypertension may need closer monitoring as these values change and should never stop existing treatment independently.
3. Who may be eligible, and who should not use it?
BMI and Asian risk
Medicines are generally considered at obesity thresholds or overweight with weight-related illness. Asian populations can develop metabolic risk at a lower BMI, so a clinician also examines waist, glucose, blood pressure, liver risk, previous interventions and personal goals.
Contraindications and important cautions
- US labelling contraindicates use with personal or family medullary thyroid carcinoma (MTC), MEN2 or serious tirzepatide hypersensitivity.
- Weight-loss treatment is not used during pregnancy; pregnancy plans and breastfeeding require individual review.
- Previous pancreatitis, gallbladder disease, severe delayed gastric emptying, kidney disease or diabetic retinopathy requires more careful assessment.
- Insulin or sulfonylureas can increase hypoglycaemia risk.
Not a cosmetic shortcut at normal weight
Without a medical indication, potential harms—including gastrointestinal illness, inadequate nutrition and loss of lean mass—may outweigh benefit. Before-and-after images rarely show the full clinical picture.
4. How is it used?
Weekly subcutaneous injection
Common sites are the abdomen, thigh or upper arm, rotating sites. Training must use the exact device supplied; do not infer technique from a different pen online.
Gradual dose escalation
US labelling starts at 2.5 mg once weekly for four weeks, then 5 mg. If required and tolerated, increases are 2.5 mg no more frequently than every four weeks, to a 15 mg maximum. Patients in Thailand must follow the Thai label and their prescription and must not self-escalate.
Missed dose
US instructions allow a missed dose within 96 hours. If more time has passed, skip it and resume the schedule, keeping at least 72 hours between two doses. Contact the clinic when uncertain.
Storage in a hot climate
Protect from light and freezing. US labelling allows up to 21 days at no more than 30°C, but a car or bag in Phuket sun may become much hotter. Follow the leaflet for the product actually received and never use a frozen or heat-damaged pen.
5. Which adverse effects matter?
Common effects
Nausea, diarrhoea, constipation, vomiting, abdominal discomfort, indigestion and appetite loss are common, particularly near initiation or escalation. Smaller lower-fat meals and adequate fluid may help.
Urgent symptoms
- Severe persistent abdominal pain, especially radiating to the back
- Repeated vomiting, inability to drink, little urine or dizziness
- Right-upper abdominal pain, fever or jaundice
- Breathing difficulty, facial or throat swelling, or severe allergy
Lean mass and “Ozempic face”
The media term is not a diagnosis and is not specific to one medicine. Rapid weight loss can reduce facial fat and lean tissue. Sufficient nutrition and resistance exercise help preserve function, with adjustment for kidney disease or other restrictions.
Oral contraceptive effectiveness
US labelling recommends non-oral contraception or an additional barrier for four weeks after initiation and every escalation because delayed gastric emptying may reduce oral contraceptive effectiveness.
6. Will weight return after stopping?
Evidence from SURMOUNT‑4
After a 36-week lead-in with 20.9% mean loss, participants switched to placebo regained 14.0% on average during the randomised 52 weeks, while those continuing tirzepatide lost a further 5.5% on average.
Is treatment long term?
There is no single duration for everyone. Benefit, adverse effects, access, health priorities and preference are reviewed. A planned stop should include nutrition, activity, monitoring and an agreed response to regain.
7. How lifestyle protects the result
Prioritise protein
When appetite falls, nutrient intake can fall too. Include a suitable protein source with nutrient-rich food at meals; the amount needs adjustment for kidney disease, age and activity.
Use resistance training
Strength work adapted to ability can help preserve lean mass and function. Monitor strength, waist and wellbeing alongside scale weight.
Use smaller meals and adequate fluid
- Eat slowly and stop at comfortable fullness.
- Choose smaller meals and less fried or fatty food.
- Sip fluid regularly in Phuket's heat.
- Avoid alcohol when nausea, vomiting or dehydration is present.
8. Using tirzepatide responsibly in Thailand
Prescription care, not a beauty product
Thai law restricts public advertising of controlled prescription medicines. This page provides education without a price, purchase link or inducement. A consultation is an assessment, not a medicine reservation.
Avoid counterfeit or improperly stored products
Unverified online pens may be counterfeit, contaminated, incorrectly dosed or heat damaged. Use a lawful clinical and pharmacy pathway, verify the registration and package, and preserve the cold chain without freezing.
Disclose PrEP, antiretroviral therapy and all other medicines
Bring a complete list including oral medicines, hormones, diabetes medicines, contraception, herbs and supplements. Delayed gastric emptying and vomiting can affect absorption or adherence for some drugs; the exact product and dose need pharmacist or clinician review.
9. Frequently asked questions
How much weight will I lose in month one?
No figure is predictable or guaranteed. Month one commonly tests tolerability at the starting dose, and fluid changes can distort short-term scale results.
Can a person without diabetes use tirzepatide?
Some tirzepatide products have approved weight-management indications in some countries for eligible people without diabetes. Thai registration and individual criteria must be checked.
Can I drink alcohol?
Alcohol may worsen nausea, dehydration or low glucose risk in some people. Avoid it during initiation and escalation and ask for advice based on your history.
Mounjaro or Ozempic—which is better?
Neither is universally better. The appropriate choice depends on indication, health conditions, other medicines, adverse effects and access, not one headline percentage.
Can I inject at home?
Potentially, after a legitimate prescription and training on the exact device, storage and sharps disposal. Never repeat a dose by guesswork after a device problem.
10. Start with an assessment, not a pen
Bring a medicine and supplement list, recent results, weight history, previous strategies, pregnancy or contraception information and goals. The clinician can compare medicine and non-medicine approaches and set measurable review points. If tirzepatide is unsuitable, another pathway may be safer.
Discuss weight-management options with PMC
Phuket Medical Clinic provides individual assessment at four locations. Book through PMC Booking and choose Laguna, Circle Clock Tower, Phuket Town or Koh Sirey.
Booking does not guarantee a tirzepatide prescription or supply. Approved indication, suitability, availability and the clinician's judgment apply.
